Pharmacists/interns collaborate with clinic providers to provide integrated DM management to underserved patient populations in Hartford, CT. Most patients are uninsured or underinsured and have poorly controlled diabetes. The interprofessional team works to optimize medication management, provide education, and connect patients to community resources and programs.
